- Board Certified: National Board of Physicians and Surgeons (NBPAS)
- Active Licenses: CA, FL, IN, KY, MI, MO, NJ, OH, PA, TX, VA, WI
- DEA: Active
- IMLC: Eligible
- One prior malpractice suit (2005), dismissed with prejudice
